Instructions for Completing Cryo Service Requests
Process
- Assemble all information needed to place your order (see below).
- Register and log in to TCP LIMS.
- From the Service Requests menu, select "Request a TCP Service".
- Search for Department "Canadian Mouse Mutant Repository (CMMR)".
- Select the appropriate Service Request form.
- Click the magnifying glass next to Principal Investigator and select the appropriate PI name.
- Fill in the remaining information on the Contact Information and User Forms using the information gathered in Step 1.
Investigators must execute two original copies of the appropriate MTA. An electronic form copy of the MTA will be forwarded to you by email once your request has been accepted. In cases where no materials are transferred (e.g. Strain Services), acceptance of the Service Agreement is required.

Information needed to complete Service Requests
Service requests must be completed in their entirety before submission. Service Request forms cannot be saved or edited by Requesters. When completing Service Requests for the CMMR, ensure that the following information is available:
- Principal Investigator name (principal investigator must be previously registered in the TCP LIMS)
- Contact (PI or Designate) name, telephone, fax, email, and mailing address.
- Billing information with payment method. For TCP Member Hospital Investigators, the cost centre to which your requests should be billed. For others, preferred payment method and purchase order number, if necessary.
- Courier name and account number.
- Mouseline Passport common name of strain (all CMMR strains have existing Mouseline Passports; if your strain is new to the TCP LIMS, complete a Mouseline Passport prior to submitting a service request).
- Sample type to be submitted and source of the sample.
- Identifying information for mice and/or samples to be submitted.
- Genotype(s) of mice and/or samples to be submitted.
- Animal housing information for mice being submitted or to where any derived mice will be shipped with name of the facility, address, veterinarian's name, telephone and email.
- Materials Transfer Agreement (MTA) will need to be completed and submitted prior to scheduling of any requests. Once you've submitted a request, we will send you a template MTA to complete and execute. In the case of some not-for-profit requests, you will be asked to accept CMMR Terms and Conditions of Use or agree to our Service Agreement rather than submitting an MTA. We will contact you as necessary for completion of these forms.
